2015 SUMMER READING PRIZE EARNING GUIDELINES

BIRTH THROUGH 5TH GRADERS

·        Library Visit Log

·        Stickers available June 16 - July 31

·        Kids visit the library six times

·        Infants/toddlers through school-age kids (K-5) may participate

·        One sticker for every day child visits

·        Only one sticker per day

·        Choose one free paperback book upon completion of 6th visit

·        Complete only one Library Visit Log during the entire 2015 summer

·        Forgot your log; ask for a dated sticker

·        Friday, July 31 is absolutely the last day that stickers and book prizes are available

·        A bonus Library Visit Record Sheet is available. Adults, set a separate goal with your child to earn an additional prize you award them.

 

3RD THROUGH 5TH GRADERS

·         Choose to participate in either 3-5 Reading Challenge OR Library Visit Record Sheet NOT BOTH

·         Reading challenge: read and date any five books between June 16 through July 31

·         Must be returned by Thursday, July 30 to be eligible for the prize drawing held Friday, July 31 Winners notified by phone on July 31

·         Give completed CHALLENGE sheet to Miss Nola or Miss Sue (10:00am to 6:00pm daily) for a food gift certificate and a paperback book choice

·         Incomplete contact information will disqualify entry from the prize drawing

·         Only one entry per participant (3rd through 5th graders)

6TH GRADERS THROUGH 18 YEAR OLDS (entering 6th grade fall 2015)

·      Reading Challenge and Game Packet

·      Read and date any five books between June 16 through July 31

·      Must be returned by Thursday, July 30 to be eligible for the prize drawings held Friday, July 31. Winners notified by phone on July 31.

·      Give completed challenge sheet and game packet to Miss Nola or Miss Sue (10:00am to 6:00pm daily) for a food gift certificate
